I was having this same problem until I turned off "Sleep mode" under Power settings. It seems that even if you set "Keep wifi on during sleep" to "Always," it will still sleep the wifi radio if you have "Sleep mode" on. I don't think this is an issue with your wifi toggler app.

Actually, my unlocked version came with headphones. I think it's only the Tmobile version that's missing them. Although I personally use a nice set of stereo bluetooth headphones and haven't even opened the stock ones.
