How do I get my balance/service end date on the screen?

The box has a square thing with a maze in it that I am to scan to download the "TracFone My Account App " but I don't know how to do that.

You should have a barcode app. If you don't, goto the Play Store and download one. Then, using the barcode app, scan the "square thing" (it's a QR or "quick response barcode") and it should ask if you want to download (or go to the page or something like that). Follow the steps to install the app.

(Once you scan the QR code, it'll become pretty evident what to do.)
