Can't update, OTA or via USB, pls help!

I'm currently on 4.4.2, rooted with TeamWin, with stock ROM.

N5 won't let me update OTA, it doesn't stick, even though it goes through the motions, reboots and all.

Finally I got around to doing a USB update, following instructions here

and made sure I used the correct 4.4.2 to 4.4.3 update first here

So am I doing it right? Like this? Power off phone, boot to recovery mode, press Install, scroll down to ROM.zip, swipe to confirm flash - yes?

Because I get an "Error flashng zip" message and a red "Failed" message - and buttons to Home, Wipe cache/davlik, and Reboot System.

What is wrong, can anyone help? Thanks much.

Factory data reset. Update should work after that.
