It simply means your signal is very poor, the network in that area is crappy at that time. In Nigeria we get that often especially in rural areas so I'm amused to see it's not something you're used to :)

The real answer is that you are using an unlocked phone on the AT&T network.

The reason it drops to H+ is because AT&T does not offer VoLTE (HD Voice, as they call it) on unlocked phones. It switches to regular GSM signal for calls while maintaining a simultaneous data line. It just isn't LTE anymore as long as you're on the call.

It has nothing to do with signal strength.

It has everything to do with AT&T wanting to be the one selling you the phone.

The only unlocked phones that AT&T will let used HD Voice, along with Wi-Fi calling, is the iPhone (because, well... Apple) and the Pixel.

Most phones made today can do VoLTE, but AT&T actively blocks the service when an IMEI sweep is done looking for non-AT&T phones on their network.

However, relief may be coming. AT&T has started allowing some unlocked Note 10s and S10s to get their "advanced calling features". So... maybe in the future it will change.
