There are a few threads about this and some are in the note 4 forum.
I think its related to uninstalling apps... it seems to move pictures to a hidden file imo.

Between these 2 processes I got all of my pictures and video back. In this day and age it is senseless to not back up you images and video to cloud as most phone are doing away with sd card any way.
As I said my images and video disappeared after unistalling apps...I think it moved my images to a hidden file...some of the apps even gave a warning about deleting media files but I didin't check the box and wham...my stuff was gone until the process above was used. Good luck .
