Can you open the S Health app and turn it off there? This support guide is for the S4, but it's probably the same for the Note 4. Try going into the S Health setting and see if you can delete the account you set up. What is S Health, and how do I use it on my Samsung Galaxy S® 4? : AT&T Cell Phones | Samsung

I just turned off the app.
what I did was go into settings > applications and select s health, clear cache, clear data, and uninstall updates.
S health should be in your apps and then just press on the 3 dots and select disable.it should then go into disabled apps or even deleted.
If it does get deleted you can always just download it again from the Samsung app store.
