why are pics I'm taking not saving properly to my sd card?

I have a Samsung Note 4 running 4.4.4. My setting on camera is to save to my sd card. I take a pic and it's there but it's blank with an error type symbol. How can I fix this is it a problem with the sd card or the phone?

It could be a bad SD card or you can try removing all the files from the SD card and then formatting it in the storage setting of the phone.

Simplest test is to pull the SD card and take a pic.... if it works, copy everything off the card, pop it in and format it and try to store a picture there again. If it fails again, it is most likely a corrupted SD card (more common that you think).
