My Samsung quick charger died, is this common?

The included charger for my Galaxy Note 4 seemed to die yesterday, and I'm wondering if this is common. I think it's dead because:
1. Using it and the included cable, my Note 4 won't charge.
2. Using the charger and a different cable, my Note 4 won't charge.
3. Using the included cable with an old USB charger, my Note 4 WILL charge.

That seems to indicate a problem with the charger, not the cable or my phone. I'm just wondering if others have run into this, and whether there might be workarounds. I honestly can't think how this can be fixed by an end-user, though, so should I just contact Samsung and try to get it replaced?

Contact them. They replaced mine due to it not charging and the charging cable getting hot.

I've seen mention of it happening before, but it doesn't appear to be terribly common, at least not based on the discussions I've seen here.

But it is a warranty item, and Sammy should have no problem popping a new charger in the mail and sending it to you. If they give you any issues, QuickCharge 2.0 chargers from quality vendors are like $10-15 on Amazon, so it won't break the bank. Not that I expect them to give you any flak about replacing a charger...
