I think it came already installed on my phone as I don't recall downloading it.

It isn't listed in app list in the application manager (under Downloaded or All) - I checked because I was going to uninstall after hearing the flashlight app malware news stories

If it came pre-installed on the device it should be listed under "all". It might be labeled differently, or bundled with another app since it's a widget. Run the widget then take a look at the running apps lists to see if its there.
