3. Camera rattles when shaken. When I turn the camera on, the rattle stops

3. Yes, it is not defective, it is the camera lens. But I don't intentionally shake it to make the rattle sound. Just use the phone normally and don't be obsessed with the rattle sound. You can test by turning on camera and then shake it, you won't hear it rattle.

Yes.

This thread is the first that I've heard of the rattle though, and I had to check to see if my phone did in fact rattle. Is it a defect? Should I be concerned?

No you should not be concerned. It is meant to be that way. If you turn on your camera and then shake it. It doesn't rattle.

Yes, both of them do.

Once I realized the rattle is due to the camera/shutter I got over it and haven't even noticed it for weeks until I saw this post.

I didn't notice it until a few days in and thought something had broke. Then I found here that it was normal. It's a bit annoying because it makes the noise when I'm tapping on the screen, which makes the phone seem cheap.

3. I hear a slight rattle if I shake the phone up and down (holding it flat/horizontally)

I assume its the vibration-spinners making the rattling sound. It's barely noticeable and only happens if I have the camera app on.

It's the floating camera lens. Virtually all camera phones have this to a lesser or greater degree (as long as the camera is not fixed focus), including, from personal experience, the iPhone 4, the iPhone 5s, GNex, GS4, and now GS5. Even if it's too quiet to hear, it's there. It is a natural side effect of the lens designs, and is not an indicator of an issue.
