Action Block dials automatically with no one near the phone!!!!!

I have set up Action Blocks on my wife’s Galaxy Z Flip mobile phone resulting in icons for each of her contacts. This makes it easier for her to use due to her physical difficulties.

Today, the phone was left on a table whilst doing other activities and without anyone being near it, the mobile dialled a relative’s number. This has apparently, we now found happened five times today.

Can anyone throw any light as to how this could occur?

Thanks for that link.
I have in fact deleted the Action Block for that particular contact and then recreated it and will see if that cures the problem, although it might be just a one off problem as it has only happened today and with only that one contact.
