Not sure if this is possible or if anyone else has these issues. I have a skin issue under where the metal and pulse sensor are on the watch. I'm a Nurse and it doesn't seem like a sweat rash.. maybe something from radiation? I've worn it every day for almost a year now.

I studied medicine but I am not a dermatologist, so, don't take my word for it. It doesn't seem to be what you call radiation burn but rather an intolerance against certain metals / alloys if not even psoriasis (check with your family for similar medical conditions - past or present).

What happens if you don't wear the watch for a week?

1. There is no radiation in wireless signals. They're all radio waves.
2. You're probably developing an allergic reaction to the device, probably from the band or the bottom of the device. Allergies can develop from anything at any time. I'm not an allergist or dermatologist, so take this info for what it's worth.

I get this too. But never from previous Casio ProTrek, Tissot or Rolex.
Might be a reaction to the case back material, stainless steel on previous watches, unknown plastic/glass on the Gear S3.

There is radiation in wireless signals, Microwave ovens use radio waves and would obviously cause serious harm to any biomatter exposed to them.

Radiation is a broad term. Microwaves use ionized radiation which can be harmful. Bluetooth, wifi, cellular do not.
