Problems with camera and video.

So I recently noticed that when I playback recorded video, there are parts where it is very pixelated and blurry. I have it set to highest quality, did a factory reset and still no solution. I have noticed that when the video is done recording it doesn't immediately save it and get prepared to shoot another video,I usually have to hit the home button because it freezes. Also when I take some pictures and try to click on them in gallery i get this icon that is attached. Any suggestions.
Had this phone for about a year and was the perfect camera until recently.
