Samsung Galaxy S4 Battery Issue

Finally I've reached a place where maybe I can get an answer, I have not found a forum or a solution to this yet and no one has been able to answer me yet.
Here's the thing, the battery has been **** for the last 3-4 months. I can barely go a full day with the battery, sometimes cant go a full day at all. I am also in school so I dont use my phone that much, the battery drops 1% when I'm on it every minute and 1% every 2-3 minutes when its not in use. Everything is turned off and I can assure you, I have factory reset the phone 5-6 times nothing; wrong physically and everything from S voice wake-up commands to smart features is ALL turned off; Sync,Google services, GPS, everything! Here are a COUPLE of screenshots:)
P.S. I'm now on KitKat(4.4.2) T-Mobile(SGH-M919) and the problem STILL PERSISTS.

Hm this is hard for me to diagnose since you don't fully recharge the battery so the chart doesn't really reset. It looks like the battery sometimes drops due to being awake which wake lock detector would tell you what is causing it..

Otherwise it looks like the phone is awake when the screen is on (which is expected) and drops accordingly since the screen is usually the biggest drain.

Here's 2 more pictures and the reason why I didn't charge it is because it would run out every couple hours even during school i had to keep charging it to even keep it alive. And I cant show any wakelock detector stats because with KitKat it doesn't work. btw as you can see on the KitKat update where the phone went 1 day 6 hours and still had 46% battery that alittle bit better, but then it starting getting bad after the update again.

It may just be your reception. How many signal bars do you normally have?
