How to switch from Verizon to T-Mobile prepaid?

I'm sure I'm one of many who are ditching Verizon (and the Galaxy Nexus) for a Nexus 4 and going prepaid. I have a new Nexus 4 and would like to leave Verizon, pay my ETF, keep my phone number, and go prepaid on T-Mobile's service. I've been scouring the net and have been unable to find really clear instructions as to how to do this, step by step. Can anybody point me in the right direction? Please forgive my ignorance. Thanks!

Ask T Mobile to port your number, and you're done. Verizon will automatically cancel your account and Bill you the termination fee.

If you want to do this online, you can. Just order a SIM card from tmobile ($.99) free shipping, once you get the sim in the mail go to tmo website and activate the sim. You'll have the option to port the number there and choose the prepaid plan you like.
