Audio/Media Issues 3rd Nexus Coming!!!

I have another Galaxy Nexus coming hope the 3rd one is the charm. First was a battery issue was only getting 5hrs out of extended battery, I'm thinking it was the actual battery that was bad but hey they told me i could just swap phones so i did. 2nd phone was a audio/media issue. Basically i was able to hear my notifications, but say if if played music or movies i couldn't hear anything unless i plugged in my headset. Im thinking its some type of hardware problem, but it also has been happening on calls where the speaker goes in and out, and where i can clearly hear the other person talking but they cant hear me; happens on speaker phone and bluetooth. At times it works ok, and i even tried factory resetting phone.
