Brand New Google Nexus wont update to 4.4.2

Hello, I recently purchased a new Google Nexus 5. However, when I go to about phone and system updates, it shows an update and I click on restart and install. However it never restarts and the system never gets updated

Is the phone rooted? Custom kernel? Custom recovery? Xposed Framework?

Any of these will keep an OTA update from getting applied automatically.
