Call getting dropped immediately after dialing a number.

This is a Nexus 4, less than a year old. The issue is - As soon as a call is dialed. The call drops off. Rebooted several times no change. Apart from this the local sim connection also shows up as roaming near the signal bar. Please help with your inputs on how to sort this out.
