Sure, as long as you have a GSM phone, aka one that takes a SIM card, and your new phone is either unlocked or on the same provider you are already on.
Just swap out the SIM card into the new phone and you are done. If the new phone takes a different size SIM card you would need to spend like 10 bucks for a new SIM from your provider.

Even Verizon will let you switch phones, as long as the new one works on the network and takes the same size SIM card.
