Just to save someone else the leg work:
There may be a way to get your msl code from the HTC Evo Design...but I wasn't able to do it without calling Boost. The lady was happy to give it to me when I explained that my computer was asking for it when I was hooking it up to the USB port. If I would have known that they would give it to me so quickly I would have saved the day spent trying to extract it.
I had a heck of a time getting my Windows 7 computer to recognize my HTC Evo Design as a modem. You have to manually install the HTC Diag Drivers and they are located here:

It really sucks because Windows 7 keeps spitting out the drivers when the device is disconnected and it took me a long time to realize this.
Even though I had my MSL password, the SPC code didn't pop up in cdma when the password was entered, so I had to manually change it.
You have to enter Diag Mode ( Dial - ##3424# , then enter MSL password ) , this lets cdma workshop recognize the device by entering Diag mode.
I was able to change my SPC code to 000000 with a program called RevSkills - The process is explained Here:

Enter password and SPC Code and Send SPC in CDMA, then your phone is unlocked.
....now if I was only able to get DFS cdma workshop to read from the HTC Evo Design......
