"Desktop Visualizer" is a great app found in the Market (Free) and it does all that and more. I hope it helps.

I believe some of the launchers allow you to change them on the home screen.

A lot of launchers allow you to customize your icons to varying degrees, yeah. I know Go Launcher at least allows changing the icon and renaming it.
