Looks to me like a defective digitizer. Not sure that any phone should look like that. I would look into a replacement of some form (warranty, insurance, etc.)
Have you tried the dreaded factory reset yet? usually this clears up most issues. If its still like this after that, then it is definitely a hardware issue.
