Display Stays On While Talking, Puts Call on Hold

Most of the time when I am talking on the phone, my display will stay on. While it is not a big deal when using the speaker, it is when I have the phone pressed up to my ear. This causes the phone to recognize any touches made to the screen. So in all cases, the put on hold button gets pressed.

Is this an issue for anyone else?
 
The opposite problem - display going off the moment a call is initiated and unable to bring it back to, for example, use the keyboard during a call - is nearly always caused by a screen protector or case which is blocking the phone's proximity sensor.

I suppose it is possible that the cause is the same but with different results for whatever reason (screen protector material, perhaps?).

My phone is completely naked: no case and no screen protector.

I just did a restart and that currently fixed the issue.
