Electronic shriek during calls

This is my second GNex to emit a piercing electronic shriek during calls. It lasts for only a second or two, is inaudible to the other party, and happens a max of once per call. I haven't experienced it on speakerphone.

Is this a phone or network problem?
 
I have had this happen twice to me while using my headphones on calls. I hadn't had this happen until I updated. Not sure what causes it but it is certainly painful to the eardrums.

My first phone was on 4.0.2 and the replacement is on 4.0.4. Neither involved headphones (ouch).
 
the same problem effects my phone. i've seen people say that it is a hardware problem and i've seen people say it is a software issue. some people say it depends on where your phone was made (china or korea). or when your phone was made (serial#) who knows. i'll go two weeks without a problem then i'll have three calls in a row where they all have the noise. it is annoying but it doesn't happen enough for me to want to return the phone. which of course means my phone is now self destructing in 3...2..1...
