Flaky 4G LTE connection

I have a Samsung Galaxy S3 that I am running on the Sprint network as does my wife. For some reason I no longer get a 4G LTE connection at home but she does. Both phones have been rooted. I've looked on-line for a solution but can't seem to find one that would work for me. Do I need to reset the phone back to factory settings and re-root or should I look to put on a custom ROM?

Sounds like you should contact your carrier - perhaps a replacement SIM card will help (or just re-seating the existing one), or maybe they just need to reprovision your device at their end.

Do you see this behavior anywhere else or just at home?

It was working at work but now it's not.

Update:

The DEC and HEX values match what is printed on the inside of phone under the battery. There is no SIM card to pull. The DEC value does not match the IMEI number and I'm not sure if it should. Not sure what the ICCID number is or how to check it.

If I need to have either Sprint or Samsung re-provision it do I need to un-root it?

I'm willing to un-root the phone to return it to stock then re-root it. I'm having trouble finding and downloading the correct firmware for my phone. It is the Samsung Galaxy S3 L710 running on Sprint. If anyone knows where I can download the firmware please let me know.

You can find the firmware here - not sure if you are running 4.4.2 or 4.3.

I'm not familiar with the DEC and HEX values that you mentioned (the Verizon variant that I have uses the IMEI number) - sorry.
