Whenever I listen to Pandora and want to turn up the volume via phone, I get a annoying pop up saying listing to loud music is bad or something. What is this pop-up? I have to then look down at my phone click OK then hit volume up... I've seen this posted before but search with no luck. Is this app related or is it android or s4. Maybe a habit from s2 I should kick? Also phones always plugged into 3.5 jack into a head unit.

That's the healthy hearing nag if you're using headphones (or anything plugged into the headphone jack). If you're using the Samsung Music player, you can turn on Automatic Volume Leveling to disable the warning. This trick doesn't work for 3rd party music players though, including Play Music. Supposedly there's an Xposed Framework mod that will disable it entirely, but I haven't tried it.

Would this still happen if it's over blue tooth? My head unit doesn't recognize my phone over USB connection. Just says (n/a usb)

Don't know about that. My BT doesn't have the A2DP profile for music streaming to test the theory either.
