GPS/Location problems?

On my Nexus S 4G, when I open maps or an app like Foursquare, it shows a place many miles from where I am. If I try to get directions it uses that wrong loaction as the starting point. I have ried it with a wifi connection and with wifi off, using 3G. The Sprint site shows excellent 4G coverage at my address - but I can't get 4G in my house or just outside it.

How can I get the GPS to set the location correctly?
 
I use foursquare a lot and I've noticed lately I have been needing to hit refresh several times before it locates accurately. At first it will say 1.5 miles away or something, and after a few refreshes it will drop down to a few hundred feet or less. Has it always been off?
 
Sometimes I can't get it to locate correctly no matter how many times I refresh. I assumed it was Foursquare, but having Google Maps come up with the same wrong location makes we wonder if there is some default location, or if there is a needed calibration of some sort.
