I'm leaving this here mainly for my future reference, but maybe it will help someone else who wants the same setup I do. I have my Google Voice and Sprint numbers integrated. I want to have the Voice app handle voicemail and Handcent (could just as easily be stock Messaging, Chomp, etc.) handle SMS and MMS. Here are the settings that got that to work for me.
On the web (www.google.com/voice) settings page, Phones tab:
In the Android Voice app:
I also told Handcent to be the default app for SMS and MMS. Using all of that, I now get a Voice notification when I get a voicemail and a Handcent notification when I get a text. SMS continues to be logged in GV in case I ever want to go back and look at some ancient conversation.
I sincerely hope that this helps someone. I had it working previously, but I've been playing around with different ROMs lately and Titanium apparently didn't copy the settings quite right. I spent a lot of time searching for answers and trying different configurations until I got it back to this. Hopefully this post will save you the trouble!

- Forward calls to this phone checked next to my mobile
- "Receive text messages on this phone" checked
- "Notify me of new voicemails via text" unchecked

- Make sure you have the latest updates to the app--I had to manually search it out on the Market as it wasn't in My Apps
- Under Settings, Sync and Notifications
- Background data and auto-sync enabled
- Receive text messages "also via the messaging app"
- Voicemail notifications checked
