Have unlock code (Captivate)...how to enter it?

I have a Captivate locked to AT&T (rooted, though I don't think it matters). I have the unlock code (paid a service for it; couldn't figure out the other methods online). Where do I enter the code?

I've booted up with no SIM card, and with an inactive non-AT&T SIM card, but don't get an option to enter the code. I just get "Not registered on Network" if I use the phone dialpad to enter any strings.

Any help?

I'll add that when booting up with a non-ATT SIM, an "Attention" notification pops up:
(SIM not provisioned MM#2)

With an "OK" button to press. Nothing happens after "OK" pressed.
