how to disable the vpn client on a galaxy note4

how does it get on the phone is it already installed on the phone or someone has to install it themselves? and what is the purpose of this app

Welcome to Android Central! It's called VPN Client? Go to the App Manager, select it, and see if there is an Uninstall button. If it's only Disable, or if the buttons are greyed out, then it's likely a preinstalled system app, and is intended to help connect to a VPN if you want (read more here: https://en.wikipedia.org/wiki/Virtual_private_network).
