I bought a Samsung S20 because Android 12 is compatible with my medical devices. The devices are NOT COMPATIBLE with Android 13. How can I make sure that an auto-update doesn't force me to Android 13?

Hello,
Do these settings on your phone,
Go to Settings > Software update > then simply disable Automatic update option, then never update your phone. It will ask for updating the software, but don't allow it.
Some Samsung don't have that option under software update menu , you can postpone it couple times when OTA comes through but eventually it will automatically update

My experience with my non-Samsung Android phones (like Pixels and Moto's), was that they only automatically updated if they were plugged in AND connected to wifi. Since we have a landline I shut my phones off every evening THEN charge them before bed and unplug them. I've never had a phone auto update, I've always chosen when I updated. My wife's Moto DID auto update when she had it on and charging on her desk while connected to our home wifi though, so her G6 is set up that way.

I read that a long time ago, but from what I remember it was an Android thing... so I don't think Sammy would be any different. They did that so people with limited data wouldn't be charged overages for the download and installation, and also so the phone wouldn't run out of juice part way through the update. That's supposedly the default setting.

I'm not sure if it's still set up that way... but it would be one roundabout way of avoiding an update ¯\_(ツ)_/¯
