If I unroot my HTC EVO 4g, do I have to re-activate it with Sprint or it will still be activated?
 
Well, one clear sign is if you are using the phone like normal. If so, then you don't have to reactivate it as it's already working. If nothing is working and you are not getting any service then you may need to reactive, but I don't think that's the case when you unroot or reflash a ROM image that doesn't have root access.
 
Your phone is always activated unless you clear out the radio settings, in which case it'll re-active once it talks with sprint's network so you should be fine.

Rooting is to modify the fone from its original state... and have nothing to do with the activation of the fone. So, unrooting the fone would pretty much do the same.
