HTC One Tradeup Proof of Purchase doubt?

I preordered the One and received it around April 24th or something. I did register for the tradeup before the initial deadline, but I simply can't figure out what to send as Proof of Purchase.

I don't have the invoice that came in the box along with the One, so that's out of the question. I have the One's box which has the UPC code, IMEI and Serial number on it. I also have the stickers that come in the box, that have the S/N, IMEI and some other numbers on it.

A valid proof of purchase is a) an original invoice with the IMEI number printed on it or b) a copy of the UPC code with the IMEI code printed on it. A copy of a UPC code is only permitted once as a valid Proof of Purchases.

What directions? Sorry if my OP wasn't clear enough. I already have a quote, and a shipping label. I just can't figure out what to send as a 'proof of purchase'.

I sent a copy of my confirmation email from Sprint, a copy of the barcodes on the box, and the packing slip (no IMEI number listed there). I finally got my gift card 2 days ago.

No today is the 14th day before my wife could return her One it so I was waiting till then to make sure she didn't want to go back to her iPhone 5. I have till June 19th to send it so I will do so after the weekend.

Once you enter your imei it has your proof of purchase that you purchased a HTC one, then send that print out along with your old phone which has your approval code and your golden...

Thanks for the suggestions, everybody. I even emailed HTC Support, and they told me the same thing:

Thank you for your inquiry.
If your invoice or sales receipt does not have the IMEI on it, you may copy the side of the box that has the IMEI # on it and include it in your package with the sales receipt.
If you have any other questions please let us know.

I took a photocopy of the back of the box which has the UPC on it, and also printed out my online Order Receipt. I emailed these along with my used phone. I hope I get my rebate as expected
