Ok my fassy had a ...let's say accident, but insurance will only replace an unusable one, so this one needs to have more of an accident.

Have it "fall" from a multi-story building! I say this because I've had an ex throw my phone from the 3rd story of an apartment building it dented a car and shattered all over the ground.. Although it was a crazy reaction it was definitely something to see! lol.

Now that is a good start....but I am worried this one might survive. It already jumped from a car going sixty, got run over and survived with just scratches and bruises..well and one app that wont work anymore

Ok I have an obscure one for you, smash the phone with a hammer, then say it fell out while you were using the cardboard bailer at work and the phone got compacted by the bailer. Just say it fell out and you didn't realize it until after you had compacted it and you were unloading the cardboard bail found pieces of the electronics and realized it was your phone.

Or you could say you went skydiving and it fell out after you pulled your chute.

Or bungee cord jumping and it fell out of your pocket and smashed on the rocks below.

Or you were mountain biking, dirt biking on motorcycles, skateboarding (insert sport of your choice here) wiped out and it took your phone out in the wreck.

Or you were at a friends party and someone threw you in the pool and your phone was in your pocket and is permanently water damaged.

But why not just say it's lost. (They always cover that)

I would suggest that you smash it with a vice, and say you dropped it as a forklift was passing you at work and it was flattened beyond all hope :-D Whatever you do please take video and share. Make sure not to mention anything about why you're doing it.

Hmmm. I'd say leave it in the pocket of your shirt and run it through the washing machine, then the dryer--on high.

The water ought to mess it up, but if not, the banging around and heat in the dryer ought to finish the job.

Put it into download mode by pulling the battery and connect usb cable while holding the volume down button. Then begin to flash a rom to the phone with Oden. While it's flashing, disconnect the cable. You're phone should now be a permanent brick.

that will only work using a full stock package and it must be during the boot or sbl portitions of the flash , I should know :-\

Just to be clear...there are no forum rules being violated here.

A good parallel for this would be free wireless tethering. We talk about that all day long. This is just another way of getting over on the carrier. They have rules for how many claims you can make for a reason. They know people aren't always honest when they file them.
