Icon on dock keeps disappearing

I noticed this before updating to 4.1 & 4.2. The far right icon on the app dock disappeared. When I replace it, it disappears again after a reboot. WTF? Anyone else have this issue and/or knows of a fix?

I applied the OTA, but the problem still occurs. Thanks.

I'm having the exact same problem. I'm on android 4.2.2, using the most current GEL launcher from the Nexus 5, and the most current Google Search app. I can't figure out why the far right dock icon keeps disappearing after every reboot. I've tried putting different app shortcuts in that position, and they always disappear. This is happening on both our 2013 and 2012 Nexus 7.

I found others on G+ w/this issue, as well. Guess we'll have to wait for a fix.
