The problem is realted to Bluetooth. I have it happen every day or so and it is always when I am in my car (Ford Focus w/ SYNC). There are two Bluetooth profiles when you connect to the Ford SYNC system (and I assume with other car bluetooth systems), "Phone" and "Media". When the phone pairs with the Bluetooth, it detects both profiles and automatically turns on the Media Player. If you do not stream music over your phone to the car, you can dissable the Media option and this should stop the player from turning on.
Settings > Wireless & Networks > Bluetooth Settings > SYNC (hold) > Options... then just uncheck the Media profile.
