Custom ROMs are the stock OS, in case of the Galaxy Nexus (Android 4.0.x) and they are modified in some way shape or form.... Could be something as simple as adding a custom battery meter and some slight image changes to different parts of the layout of the OS (example changing colors of different pieces of the OS) to more advanced things like adding new options to the OS that didn't exist before, overall theme (visual changes) and behind the scene coding changes to hack or modify things away from Google's original setup. Are the custom ROM's the same as an OEM skin? What is the difference?
