Keypad error when making entry

Please help. When i make a phone call, when ask to press an extension to an office or enter a drug prescription refill, I get an error message as if i enter a wrong number. I tried doing this to 4 different locations.

This sounds really bizarre. Has anyone encountered this similar problem. Appreciate any help. Thank You
