Loud Speaker on automatically when making or answering calls

I've got an issue where the loud speaker is automatically enabled when I make or answer a call on the phone.

I've tried clearing our the data for the phone app.

I'm completely stock 4.22.

It's really annoying, has anyone else had this issue and if so how can I solve the problem?

I'm facing the same issue. When i receive any calls it directly goes on speaker. Even though the screen shows its on phone. I have to tap the speaker icon twice to bring it back on phone. If you have bought your phone locally then you should contact google and RMA it. I'm in India and i bought this from USA so i'm stuck with this. Even after two system updates its still the same. I'm going to reset the phone one i back up my data in hope that it would fix it.

Factory reset... or load latest factory images might do the trick... 4.4.2 images are out now on Google.
