Media volume is LOUD

Any ideas on how to control media volumes? For example, when using Facebook on a media posting, video can be played just fine. However, volume is always on full blast and using the volume button to lower the volume doesn't work.

In the settings, media volume is one step above mute. This setting doesn't affect any of the media volume during playback. Any ideas beside covering the speaker with the palm of my hand?
 
Yes, it happened on all media files being played on the device. It always on loudest setting and no way to turn it down.

However, now it is resolved! I followed the instruction to go into Safe Mode, tried playing a media file in Safe Mode and volume is controllable. So, rebooted back to normal mode and it works now.

Thanks! Wife is happy now that she can control the volume when playing medias. She has been bugging me for the past month since I got her the phone.
 
Great! Although the fact that Safe Mode seemed to fix it does suggest that a 3rd party app you installed was causing it. So don't be surprised if it happens again. In that case, you'd have to uninstall apps one by one until that problem disappears. It's tedious, but unfortunately there isn't an easy way to pinpoint the culprit.
