Media volume suddenly increases?

I've noticed that at times the media volume suddenly increases to the maximum level. I was playing 'Spider-man Unlimited' all the three times that this has happened... I'm on Lollipop 5.0.2

EDIT: Something weird happened again. I was playing the game and had my 'Media volume' down to 0. In between the game I switched to the Gmail app to reply to some emails and to check my WhatsApp and when I returned to the game, the media volume was on full...

Is this a Lollipop problem or happening in general?

Doesn't matter, I'm returning the phone and getting a refund...

Scratch that, it's happening on my replacement moto g2 as well now...
