My phone went dead after battery completely drained, what do I do?

I have an honor 4x phone bought in year 2015, my phone was working fine until last week. From 6 days my phone had been jumping charge percentage and get discharge quickly. But last night when it was at 13% I put it for charging and after couple of hours it was still at 13% and as soon as I unplugged the battery went to 0% within a second and phone is completely dead and drained(all this while it is still kept charging for hours and charging light is on).
