Need help with Audio

Every time I play any sort of audio through headphones the voice is very low and the music is considerably louder, I am getting a sort of externalised effect through my headphones .I checked if the concert hall effect was on, it was off I tried Google play music and it is also the same, it happens on YouTube also. The effect and distortion only happen when I use plug ins, it is not there when I use my phone speaker or bluetooth speaker. Any help would be greatly appreciated
