Nexus 10 losing charge.

My nexus 10 has been acting up recently, and I'm not sure what is going on. I have tried doing a soft reset, but to no avail. Basically even when I have my nexus plugged in, it is still losing charge. It's been pretty bad, and all of the other solutions I have found on this thread have not helped. The only things I have not tried are booting into recovery mode or factory reset, which I would really like to avoid. I have a screenshot of the battery graph, and I will try to attach it (I'm kind of a newbie...) Thanks for any and all help!

Are you using your tablet while it is charging or is it sitting idle with the screen sleeping?

Are you charging using the ?wall wart? [plug] that came with the device? Or plugged into a PC or other charger? Charging from a PC will not provide the same power output as the ?wall wart?.
