Phone Can't Read SD Card

I have a 16G micro SD card in my Touch which I had in my Evo 3D. I have many apps and photos on it. When I go into settings>applications>manage appps and select SD card, nothing shows on the card like it does on my 3D phone. How can I access info on the card from the phone? I don't want to have to download the apps again if I can just access them from the card.

You always have to reinstall the apps on a new phone. Did you use AppBrain or a backup program?
