Somewhere something duplicate with that media file in apps list , check hidden files you stop using One Plus gallery but check its garbage bin what is that app reporting?I checked and can't find any.

How about google photos, you use that?I enabled the app specifically to check for that and there was nothing there.
Go over once more the folders in file manager, if still nothing and disc storage might have to talk to One Plus and maybe get device under warranty exchange but ill check my One plus device tomorrow , maybe javier chime in more tomorrow also. But it definitely sounds like duplicate reporting.I do not use Google photo.
I checked the thumbnail folder. There were 59 items. The size difference didn't register.
Ugh. I've seen this problem reported a few times and in some cases the problem was the stock Gallery app. In your case it seems like double reporting the same storage. Just in case, are you using any third party Messages app (Textra, WhatsApp, Telegram ...)?I have disabled the Gallery app on this device because I use a different photo organizer (whose cache I have cleared). I enabled it to see if there's any trash to clear, but there isn't.
