Phone not reporting proper battery charge

I purchased a package that included 2 spare batteries and a wall charger for the batteries. I swap the battery out daily on my phone, and I immediately put it on the charger to charge until I swap out the battery the next day. Sometimes when I put the battery in the phone it doesn't show 100%. Yesterday I was furious that it showed 8% charge, so I attempted to charge my phone on the way home from work. It wouldn't charge for anything. I received a call, and my phone actually powered off in the middle of the phone call due to battery being dead. While it was off, the battery meter said 2%, and wouldn't go up. I thought I had a bad charger in my car. I attempted to charge at home with a known good charger, same thing.

I pulled the battery, replaced with the same one. Showed 100% charge that time. So it make sense that it wouldn't charge because it was already full. But it has me curious why it wouldn't show 100% charge when I first put the freshly charged battery in?

Anyone else experience this? Any solution?
 
The phone doesn't always realize you put in a new battery, it just thinks you restarted the phone or pulled out the battery and put the same one in. Someone else had a similar problem. I'm not sure of a solution, however
