Phone taken over

I have done a factory reset.
Have scanned for viruses.
Have eliminated questionable apps.

But still have a phone that seems to be controlled all on it's own.

Nexus 4 running 5.1.1

Could use some help. There are times when the phone acts normal, then all of a sudden apps start opening, and I can't even get past my password lock because what ever it is, is backing my code out so that I can't even access the rest of my phone functions.

Really sounds like the digitizer has gone bad, or that something is interfering with it.

Before going for repairs or start shopping for a new phone, try this;

Remove any screen protectors and cases
Clean your phone screen with some glasses cleaning wipes
Try it again with clean dry hands.

if the issue is still there, then the screen sensors are toast, and given that the N4 is a little long in the tooth, you really should get a new phone.
