Bought the S4 full retail at a Verizon store because I didn't want to lose my unlimited data. By the time I was interested in getting around to unlocking the bootloader and throwing custom roms on it, Verizon already released the update that squashed that hope.

Now today I randomly rebooted my phone due to having a SD card issue and it is showing the little unlock symbol and the word Custom at boot.. As far as I am aware I never took the steps to unlock it... Anyone see this before?

OP is referring to the bootloader being unlocked, which is different.

To the OP, I'm honestly not sure...I know different things can trigger that screen, but if you truly have not modified the phone (rooted it, installed custom recovery, etc), then it probably isn't anything to worry about.

Thanks guys. A few more reboots and the issue seemed to resolve itself... no more unlock and custom on my boot screen... weird..

The "Custom" and unlocked padlock are the result of the phone thinking something is non-standard.
Multiple things can trigger it.
My phone rooted, with xposed framework installed along with a bunch of other stuff has never thrown up that screen, FWIW.
The screen doesn't mean much. You can be rooted and not have the screen or be un-rooted and see the screen.
I don't know what the formula is.
