Removing "SIM Toolkit" version 6.0.1

Several years ago, I bought a SIM card in Serbia for my Samsung Galaxy Nexus. It is a data only account and I still use it every year. For some reason, the technician at Telekom Serbia installed this app. I don't use it and have never used it. When I bought my Nexus 5 in the fall of 2013 this somehow installed from a backup on to the new phone when I activated it. (Note, the phone had a T-Mobile SIM card in it when I activated.) Question: How do I completely remove this app? It does not show in the app drawer, but it does show in the list of apps in the settings screen. My only options on that screen are "Disable" & Force Stop". I have turned off all permissions (running Android 6.0.1) It starts running whenever I reboot the phone and does use some memory and data.
